i don't know if this has already been said, but this is what i believe happened.
I think that the whole movie is a tangent, imaginary world until the last few minutes. It is like a dream that slowly fades after waking, right? i also think that the time sequences in the movie are twisted and not quite in order, because almost the whole movie is a dream to everyone. This would explain why Donnie's girlfriend asks "What happened to your neck? It's bleeding." days before they have the encounter at Grandma Death's house (the guy with the knife cutting his neck). Also, this would explain why when Frank is at the movie theater and takes off his mask, his eye is gone and blood is pouring from his empty socket before he was shot by Donnie. Then at the end, you see Frank in his human form lying on the bed, feeling his eye, wondering if the whole first 9/10 of the movie really happened. Also, the way everything played out in the end, it was showing you the plane that his mother and sister were on in the tangent (the dream) and it was also showing you Donnie in his room smiling in the real-time world. He knew that if he let himself die, none of the things that COULD happen (tangent) would happen. So he laid down his life so that his girlfriend, mother, and little sister would all live in the end. This also explains why his girlfriend said he didn't know Donnie yet at the very end. Because the whole beginning of the movie was not reality. She came to his school I think the day after the jet engine fell in his room. Thus, they had not met, and she would stay alive.
