Dr. Thurmon and the end of the world

while reading the dialogue betw'n Dr. Thurmon and Donnie, a thought occurred. Donnie says, the sky is going to open up, which she reacts to. but what if her interpretation of his statement is that Donnie thinks he is going insane: the sky will open up, of course causing "the world" and "the law" (sanity) to fall away, leaving only Donnie, his memories, Frank, whom Donnie has already described as imaginary, and madness. [not sure how much i go along w/this myself; just throwing it out here as another possibility.]
as it is, Dr. Thurmon does seem to be in on what's happening, more than just as Donnie's doctor.
