There is a possible direct relevance in the historical setting of the movie during the Bush/Dukakis presidential campaign. Two issues dominated the latter stages of the campaign, in my understanding. One was the Willie Horton business - blatant racist exploitation of 'law & order' fears by the Republicans. The other was attacks on Dukakis for alleged lack of patriotism. One part of this was that as governor of Massachusetts Dukakis vetoed a law that would require students to pledge allegiance to the flag (on advice that it would be unconstitutional). So, the flag was an issue in the campaign, even though it's unlikely that many DD viewers would be aware of that specific connection. And it is, of course, such a loaded symbol that it always has potential significance of this kind. I thought it was notable that Karen Pomeroy also has her own flag, among the belongings she takes after being sacked, despite the fact that she's clearly placed on the 'liberal' axis of the film (her and Monnitoff vs. Farmer, Cunningham, etc.). Seems to me to be evidence of the relative complexity of character found in the film.
Whether the fact that the jet engine crashes through the roof where the flag was has any particular symbolic significance seems like one of those aspects of DD that's open to various interpretations. It might also just have been a way of drawing attention to the somewhat unusual image that looks up to the roof of Donnie's bedroom after he leaves the frame during the scene where we first see him go out to meet Frank (prefiguring, like the upward-angle shot of the chandelier in the entrance hall that follows, the event to come).

just throwing this out there 12 years later, but if you look at he movie poster for DD there is an american flag in the ears of Frank...
